This is an internal form used by operators of coin presses for PROOF COINS. It is titled U.S. ASSAY OFFICE, CD-1025 / (10/78). It has spaces for Operator, Press, Shift, Denomination, Date, Tray #, Ozs.,(Standard Piece Count), Part II; (Completed by Press Reviewer), Number of Condemned Pieces (Actual Count), Number of Good Pieces (Actual Count), Reason for Condemned, Date, Inspector. Appears to be from copy by emloyees (as it is not straight) on a mimeograph machine. I see two staple holes that came out black in the upper left corner. Measures 8.5 by 5.5 inches. I believe there were very few Assay Offices open and this one is from the San Francisco Assay Office which later returned to status of a US Mint. See my other lot of a pass from SF AO (San Francisco Assay Office) for a (blank) Outside Pass from around 1996. This is a POSTPAID LOT in the USA as are MANY of my Ebay Lots!!!!!!
